Job summary
To assist the Secondary Assessment Team Manager in the delivery of a quality care to patients with enduring and complex mental health needs. This will involve working autonomously in the community and clinic setting under the supervision and guidance of the Team Manager.
The post holder will be expected to provide assessment, monitoring, therapeutic intervention and treatment .Working closely with patients, families and wider MDT to provide enhanced risk assessment and formulate Care and Treatment Plans under the Mental Health Measure for Wales 2010.
Main duties of the job
We are seeking an enthusiastic, dynamic and clinically creative individual to work as a Band5 community psychiatric nurse in our busy Secondary care Community mental health team working with older adults with complex and enduring mental health needs. You will need to have the relevant professional qualification's and the related clinical experience, this post needs an individual who is highly motivated and able to work autonomously.
You willneed to possess excellent interpersonal skills and qualities and be committed to the on-going development of the service, with a person centred focus. Youneed tohave excellent all round client centred skills together with a genuine desire to work with this client group.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Swansea Bay University Health Board (formerly ABMU) was created on 1 April 2019, after responsibility for providing healthcare services in the Bridgend County Borough Council area passed from ABMU to the new Cwm Taf Morgannwg University Health Board.
Swansea Bay UHB covers a population of around 390,000 in the Neath Port Talbot and Swansea areas and we have a budget of around £1bn. The Health Board employs approximately 12,500 staff.
