Job summary
The Assertive Outreach Service provides care and support in the community for patients with severe and enduring mental health diagnosis that require a higher level of support to remain living as independently as possible.
You will work with families and carers where appropriate to ensure persons maintain maximum independence and autonomy over their day to day activities.
The post-holder will be an integral member of the Assertive Outreach Team. This is a multi-disciplinary team and the Assertive Outreach Worker will be responsible for the assessment, planning, implementing, recording and reviewing of a comprehensive package of care in conjunction with other team members.
Main duties of the job
The post-holder will manage a caseload of clients with severe long-term mental illness, liaising with the GP, Social Service, the voluntary sector and carers to ensure the delivery of a comprehensive and user-focused package of services seven days a week. The focus of the work will be in the clients own home environment and community placements and in alternatives to hospital admission. If the client goes into hospital, the Assertive Outreach Worker will be expected to ensure continuity of care in providing a service during the hospital admission.

You will need to be self-motivated, confident and have a positive outlook. This role will suit an autonomous practitioner who can work solidly as a member of a team. It is a small team and communication is central to the role. You will be required to support all patients under the service as well as those patients allocated to you. It is a very much 'whole team' approach to the service.
The service provides care to patients under both the North and South CMHT's in Neath Port Talbot.
There is a good balance of hands on work / paperwork and involves daily travel across the geographical area.
