Job summary

The Spinal Hub is based at Morriston Hospital. It consists of a 98-bedded Unit as part of the wider T&O and spinal surgical directorate. A separate unspecialised elective spinal unit in Neath Port Talbot is also being established.

There are 6 Consultant Spinal Surgeons and 21 Consultant Orthopaedic Surgeons staff the Unit.

The on call T&O tiers cover unscheduled/emergency spinal referrals. The senior spinal fellow does not participate in the T&O on call rota. If required, spinal admissions are referred onwards to the on call spinal surgical consultant.

The Spinal Service in Morriston Hospital covers all aspects of elective adult spinal surgery for the treatment of degenerative conditions and adult deformity. The unit also manages spinal trauma, tumour (intradural and MSCC) and infection. A wide range of open and contemporary minimally invasive procedures are performed from occiput to sacrum via posterior, lateral and anterior approaches. The unit commissioned in 2017 the first and currently only Spinal 3D navigation system in Wales and has vast experience in complex navigation procedures. Endoscopic spinal surgery has recently been commissioned in the non-complex elective spinal partner hospital in Neath Port Talbot. The successful candidate will therefore have excellent exposure to a wide range of spinal pathology, management strategies and surgical procedures/ techniques.

Main duties of the job

The senior clinical fellow will be trained appropriate to their level of experience but it is expected that the fellow will be at an ST3+ equivalent level. The fellowship would be appropriate for peri-CCT trainees in the final 1-2 years of training prior to Consultant appointment.

As the sole senior fellow you will be expected to contribute significantly to the running of the department including leading the daily spinal ward round attended by the Spinal nurse practitioners, Physician Associates and junior doctors. You will also be expected to lead the research and audit projects of the department.

You will have daily theatre access and therefore achieve unparalleled operative experience. The department is unique in that it has full integration of orthopaedic spinal fellowship and neurosurgically trained consultants working together hence with a variety of skill sets.

About us

The Spinal Surgical Unit is part of the Trauma and Orthopaedic directorate and more broadly, the Surgical Service Group which includes General Surgery, Vascular, Urology, Head and Neck, Burns and Plastics, Cardiology and Cardiothoracic Surgery. The Service Group is led by an Associate Medical Director, Associate Service Director and a Head of Nursing, supported by Clinical Leads.

The Trauma and Orthopaedic service at Swansea Bay University Health Board provides a near comprehensive Orthopaedic service for its catchment population.

Morriston Hospital is an 850-bedded University Teaching Hospital situated in the North of Swansea five miles from the City Centre. The hospital is a major Accident and Emergency Unit for West Glamorgan and has a large ITU. The Accident and Emergency Department is separately staffed and there are no direct casualty duties involved in this post.

The Spinal Unit has a world class operating theatre housed in the main theatre block, which is in the nucleus-designed unit adjacent to the Orthopaedic Wards. There are a further 2 elective orthopaedic and 1-2 trauma theatres each day.

There are outpatient and plaster room facilities for fracture and orthopaedic work.

The duties will be undertaken at Morriston Hospital unless the Consultant for whom the post-holder works specially designates that he/she helps at other hospitals e.g. NPT or outsourcing Hospitals. The duties will be tailored to the training needs of the fellow but will include:-

  • Leading the daily spinal ward round attended by the Spinal nurse practitioners and junior Doctors. The successful candidate will be expected to be competent at recognising and managing medically and surgically deteriorating post-operative patients and be able to co-ordinate their care in conjunction with the nursing and senior medical team members. The fellow will be closely supported by the consultant body.
  • Integrate with the Trauma and Orthopaedic department to ensure smooth management of unscheduled spinal patients admitted under their care. The candidate will be expected to attend the Trauma and Orthopaedic post take morning XR meetings with the on call spinal surgical consultant.
  • The careful preoperative preparation of patients for theatre. The Spinal Unit has a robust preassessment process but the Fellow will be expected to review all pre-operative pts on the day of surgery to ensure smooth running of the theatre list.
  • Attendance in Spinal theatre. The post holder will attend theatre and gain excellent exposure and opportunity to develop specific spinal surgical skills (appropriate to the level of experience of the fellow). Although this is a non-recognised training post the fellow will be assessed and monitored through via their logbook and ISCP competencies.
  • Attending Outpatient and Spinal Fracture Clinics under supervision of the Spinal Consultants.
  • Running the Spinal MDTs.
  • Attending the UHW Cardiff MDT and providing an interface between the 2 departments.
  • The successful candidate is expected to complete an audit cycle and submit a paper for podium presentation / journal publication within the 6 12 month period.
  • Other duties that are recommended by the Consultant.
