Job summary

To work effectively within a multidisciplinary team setting, across sites in the community, service user's homes, support agencies and hospital sites. The post is rotational between Assessment Services, Home Treatment Team, Psychiatric Liaison Services and Acute Adult In-patient Services. To provide a high quality of care to individuals with complex mental health and social problems. He /she will manage a group of service users within the guidelines of the Care Programmed Approach. To provide Clinical Supervision to junior staff. To be actively involved in the promotion and implementation of the KSF framework with a commitment to annual development reviews and continuous development of the Service/Individuals. Take a lead role in the day to day running of the team/service delivery. Shift working across 24hrs, 7 days a week.

Main duties of the job

We are currently looking to recruit a qualified RMN to join our mental health liaison team.

You will become part of a developing service, providing mental health assessment and care within the general hospitals in Swansea and Neath Port Talbot.

The mental health liaison team provides responsive mental health assessment and support to the emergency department and inpatient wards for patients from age 18 upwards, including older adults.

If you would like to be part of an exciting PLAN accredited multi disciplinary service, and have a track record of enthusiasm and a commitment to the development of nursing practice and service development, then we are very keen to hear from you.

You will hold a registered mental health nursing qualification and be able to demonstrate post-registration experience at Band 5, or above. You will need to provide evidence of continual professional development.

You will hold a sound working knowledge of common mental health disorders, the Mental Health Act and Mental Capacity Act. A sound knowledge of the interface between physical and mental health is essential as well as experience of working with older adults, in any setting.

You must have very good communication and interpersonal skills. You must be confident and motivated, flexible and be able to show initiative in an environment which can be demanding.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Ensure that all patients have an up to date Risk Assessment in accordance with Local Policy.

Collaborate with patients to provide therapeutic interventions by means of assessment and ongoing treatment. Design individual care plans in line with risk assessment and psychosocial needs and physical health care needs reflecting the patients personnel goals and aspirations.

Undertake assessments of clients with complex mental health problems identifying appropriate care pathway.
