Job summary

This is a fixed term position for 2 years to commence in August 2022. This will be subject to the successful applicant being granted out-of-program (OOP) time by their training programme director (TPD).

This post is at ST level and the successful applicant will join the endocrine surgical team at Morriston and Singleton Hospitals. This post will be a joint appointment between SBUHB and HEIW, with supervision from Prof Wyn Lewis (Head of School of Surgery, HEIW) and Mr. Richard Egan (TPD for core surgical training, HEIW).

The clinical commitment for this job will be 20% of a normal working week, with the successful applicant expected to join the General Surgery & Endocrine team for theatre lists and out-patient clinics where required. There will be training opportunities for all clinical commitments, as well as some service commitments. There is also a full on-call commitment to be undertaken at Morriston Hospital, on a rota of 1:11.

Main duties of the job

The successful candidate will be a member of the General & Endocrine Surgery team with shared care of patients and shared theatre lists with full flexibility. The Endocrine team provides specialist endocrine surgery services to SBUHB, HDUHB, and tertiary support to Cwm Taf HB, which includes referrals of patients with advanced thyroid cancer and adrenal pathology. Adrenal and thyroid cancer patients are discussed at a monthly MDT, held in UHW, Cardiff.

The endocrine team consists of 2 endocrine surgeons (RE, DSC [part-time in SB]) who provide a comprehensive thyroid, parathyroid and adrenal service. The annual caseload is approximately 200 cases (110 thyroid, 80 parathyroid, 10 adrenal). The service is supported by a Macmillan appointed CNS, full secretarial support, and input from allied services such as endocrinology, pathology and oncology, all of which are based within SBUHB.

The hospital component encompasses a Coronary Care Unit, several General Medical specialties, a well-developed Trauma and Orthopaedic service & a range of General Surgical specialties.

Job responsibilities

MAIN DUTIES AND RESPONSIBILITIES

Clinical

You will provide a clinical service in conjunction with your colleagues under the direction of a Consultant Surgeon, which will include the responsibility for the prevention, diagnosis and treatment of illness.

You will be required to contribute to the general surgical inpatients, day cases and out patients, with an appropriate level of consultant supervision according to your level of experience.

You will be expected to be able to undertake simple and routine general surgical procedures independently.

You will be expected to work within existing clinical guidelines in the directorate.

You will be expected to cover for absent colleagues and in exceptional unforeseencircumstances may have to perform additional duties to ensure the safe running of the department. Where such duties take place outside your normal contracted duties, you will receive either an equivalent off duty period or will be remunerated appropriately.

You will be required to participate in the Health Boards risk management process and in clinical audit and other clinical governance activities within the department, the directorate and the Health Board. Adherence to Caldicott Principles is mandatory.

You will be required to participate in the Health Boards Appraisal Scheme in conjunction with your Supervising Consultant, to identify training and development needs. You will be expected to participate in personal and professional development to fulfil Royal College CME requirements.

You will provide advice to clinicians in hospital and primary care, originating either from telephone calls or ward referrals.

You will be responsible for undertaking the administrative duties associated with the care of patients and the administration of the department.

You will be required to participate and contribute to Clinical Audit in line with the Health Boards policy and to undertake continuing medical education in line with recommendations for the Royal College of Surgeons.

The Ionising Radiation (Medical Exposure) Regulations 2000 impose a legal responsibility on Health Boards for all staff who refer patients for medical radiation exposure such as diagnostic x-rays to supply sufficient data to enable those considering the request to decide whether the procedure is justified.

The appointee will be required to maintain his/her private residence in contact with the public telephone service.

Travel as necessary between hospitals/clinics is likely to be required to undertake duties and a full UK driving license is mandatory for this purpose

Subject to the terms and conditions of service the post holder will be expected to observe policies and procedures of the Trust drawn up in consultation with the professions, where they involve clinical matters.

There must be no conflict of interest between NHS work and private work. All Doctors undertaking private practice must therefore demonstrate that they are fulfilling their NHS commitments.

The appointee is entitled to annual leave in-keeping with the all Wales policy and based on previous NHS service. Applications for annual leave must be submitted six weeks before the leave is taken. It must also be requested having first taken into account your colleagues leave so as to enable adequate clinical cover is maintained.

Education and Training

We place great importance on our University status and have strong educational links to the Colleges of Medicine in Swansea and Cardiff, and the College of Human and Health Science in Swansea and you will be responsible for carrying out training, teaching, examination of healthcare staff and accreditation duties as required and for contributing to and participating in postgraduate and continuing medical education activity, locally and sometimes nationally.

A staff Education Centre at Morriston Hospital, shared with the College of Medicine, with raked lecture theatres, seminar rooms and common rooms opened in 2015 as part of a substantial investment on the site to replace older facilities. Singleton Hospital has a well-stocked library and seminar rooms for staff as well as education facilities for the College of Medicine in its own facilities.

You will participate in the specialtys postgraduate teaching sessions.

Participate actively in the departments undergraduate teaching programme.

Welcome medical students into learning environments with specific the approval of any patients involved.

Engage and involve medical students in your clinical activities, where possible and appropriate.

Provide informal and bedside teaching to undergraduate medical students where appropriate, particularly during ward rounds, in the emergency department, during outpatient clinics and during operations and procedures.

Give feedback to medical students.

Participate in undergraduate examinations, particularly clinical examinations for Swansea and Cardiff medical students if requested to do so.

Research

It is anticipated that the successful candidate will undertake research with a view to obtaining a higher degree (PhD or MD). The topic of this PhD is open to negotiation but is likely to have a basis in surgical education. The successful candidate will be expected to develop a solid grounding in research methodology and ethics. This will include undertaking a GCP course, the use of statistical software packages and working as part of a wider research team (Welsh Surgical Research Initiative).

During a normal working week, 80% of the time will be allocated to research and it is expected that several peer reviewed journal articles will be published during the 2-year post. Research commitments will include regular meetings with supervisors (Mr Richard Egan and Prof Wyn Lewis), and presentation of research outputs at national and international conferences. Although this post is fully funded, and will come with study leave expenses in-keeping with the grade of training, additional funding to support research projects and scholarly activities will be sought during the post.

The successful candidate will be expected to liaise and work with our research partners, including the Universities of Swansea, Cardiff, Bangor and South Wales, the University of California, San Francisco, and The British Army.
