An exciting opportunity has arisen for an enthusiastic and motivated pharmacist with significant experience in the specialty of Thrombosis and Anticoagulation to become our Health Board wide Principal Pharmacist for Thrombosis and Anticoagulation.
We are looking for a highly motivated, intuitive leader and independent thinker, who steps up to a challenge and manages change effectively. Our ideal candidate thrives on collaborative working with a range of stakeholders, including; healthcare professionals, financial and directorate managers, people services and the wider Pharmacy team.
You will be a key member of the multi-disciplinary team within the specialty of thrombosis and anticoagulation, actively participating in the Health Board thrombosis and Anticoagulation Steering Group and leading on operational sub-groups. You will be responsible for leading on, providing and developing an anticoagulation dosing and monitoring service across the Health Board.
Main duties of the job
The post holder will be the Principal Pharmacist, responsible for UHB wide thrombosis and anticoagulation services. They will be responsible for providing a safe and effective clinical pharmacy service to the Specialty and Directorate.
They will provide clinical and professional leadership and support to the CTMUHB Thrombosis and Anticoagulation (T&A) committee, and other UHB strategic and operational committees/groups as required.
The post holder is responsible for managing, leading, developing and delivering specialist thrombosis and anticoagulation services across CTMUHB.
They will support the clinical haematology staff in the acute sites, and GP colleagues in primary care in risk assessment of the population and clinical mitigation of thrombosis risk.
They will lead on assuring the safety and sustainability of anticoagulation services by actively supporting the prescribing of anticoagulation including patient review clinics.
