Job summary
The post holder will work as a member of the District Nursing Service under the direction and supervision of the Team Leader (District Nursing).
The post holder will contribute to the implementation of nursing care as assessed and prescribed by the Registered Nurse. This will include working in a multi-disciplinary and multi-agency arena alongside the primary health care team. The post holder will perform a variety of tasks for the District Nursing teams in the geographical area.
The post holder will hold Level 3 NVQ/QSF in a health care associated certificate or possess skills, knowledge and experience to equivalent level or be willing to undertake the course.
Main duties of the job
The post holder will be required to undertake a range of clinical and clerical activities as delegated by the Caseload holder District Nursing. These activities will be carried out in a variety of settings including patients' homes, clinic settings, GP surgeries, Residential and Nursing Homes.
The post holder will also contribute to the maintenance of medical and surgical stock levels adhering to the agreed minimum and maximum stock levels required for the District Nursing Team.
The Community Health Care Support Worker will be responsible for providing support to the District Nursing Team in the day to day management of multi-disciplinary/multi-agency packages. These packages consist of scheduled and unscheduled care for patients with multiple, complex and palliative care needs. This will ensure that an efficient and effective service is delivered to the practice population.
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn't mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn't 'fluency', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
