Job summary
The post holder will work as a member of the District Nursing Service under the direction and supervision of the Team Leader/Deputy Team Leader and Registered Nurse, providing an overnight service for patients within the CTUMHB footprint.
The post holder will contribute to the implementation of essential nursing care as assessed and prescribed by the Registered Nurse. This will include working in a multi-disciplinary and multi-agency arena alongside the primary health care team.
The post holder will acknowledge their limitations and actively seek advice as necessary.
Due to the community working nature of the role, you will need to be able to travel around the Health Board in a timely manner.

Skills & Responsibilities
The Band 2 Health Care Support Worker will be responsible for providing support to the District Nursing Team in the day to day management of multidisciplinary/multi-agency packages. These packages consist of scheduled and unscheduled care for patients with multiple, complex and palliative care needs. This will ensure that an efficient and effective service is delivered to the practice population.
All patient activities will be delegated by the Team Leader/District Nurse/Registered Nurse. He/she will then be responsible for prioritising these activities unless otherwise specified by the delegating Nurse.
The post-holder must hold a level 2 NVQ or QSF in a health care related qualification or be prepared to work towards this qualification. He/she must be able to demonstrate evidence of additional or alternative training/experience and will be required to complete the essential Core Skills Training.
The Health Care Support Worker must recognise and work within the limits of his/her competence actively seeking the appropriate skills for self as well as other members of the nursing team. .
To assist the Registered Nurse and other Health Care Support Workers to provide nursing care encompassing a variety of clinical interventions to all adult patients including the transitional cohort as delegated by Team Leader/District Nurse/Registered Nurse. The anticipated outcome in all cases will be a promotion of quality of life and end-of-life care, maximising independence and minimising the effects of debilitating conditions.
Recognise deterioration/increased need of patients, and escalate concerns to the District Nurse/Team Leader/Registered Nurse as soon as possible to enable prompt intervention and reassessment of their needs as appropriate. He/she must be able to identify situations where prioritisation is essential in determining the need for a call to the GP or emergency services as required, e.g. a patient in a collapsed state.
As required, contribute to the nursing assessment of patients to assist the District Nurse/Team Leader/Registered Nurse to complete the relevant documentation for consideration of Continuing Health Care/NHS Funded Nursing Care of patients with complex needs.
Demonstrate communication and negotiation skills in situations which may be highly sensitive and emotive. All communication will be underpinned by the principles of data protection, dignity and confidentiality.
To answer the telephone, record messages accurately and ensure the information is received by the Registered Nurse.
Adopt appropriate verbal, non-verbal, written and electronic communication skills. Be open and transparent and adhere to the standards documented in the Code of Conduct for Health Care Support Workers in NHS Wales (WG 2011)
As delegated by the Team Leader/District Nurse, source patients/relatives/carers views and feedback on their experience of District Nursing Services in order to influence and inform future service provision.
