Job summary

The post holder will follow standard operating procedures and contribute to the aims and outcomes of the Community Children's Nursing Service by providing support and assistance to the Practice Development Nurse and Community Children's Nurses working within the Children's Continuing Care Team.

The post holder will, following completion of relevant competencies, plan and prioritise prescribed patient/carer/staff training within the community caseload for patients with complex health needs. Supervision from Registered Nurses will be available for reference and guidance.

The post holder will enable, support and assist carers and staff who require training to perform care tasks which are required to meet the child/young person's (CYP) health needs.

Main duties of the job

Work under own initiative to plan and prioritise required training, with guidance and support from the Practice Development Nurse, in accordance with clinical protocols.

Carry out review, and assist with initial consultations to identify training needs of carers/staff for Children and Young People (CYP) with complex health needs.

Monitor the progress of training compliance of each staff member/carer, and report to Practice Development Nurse using clear communication skills.

Encourage, support, enable, supervise and assist new staff to the CYPs care in gaining competence against care plan competencies. These Children and Young People will have a range of medical conditions and will be following clinical care plans/protocols. Have a flexible approach to work as dictated by the service, which will involve supporting new staff within continuing care packages.

Carry out assessment of competence, within own competence of staff and carers.

Provide condition related information to the CYP, carers, health care professionals and other parties overcoming any communication difficulties and following agreed protocols in line with training delivered.

Communicate with CYP and/or carers in order to review and advise on the training plan under direction of Practice Development Nurse/Registered Nurse.
