An exciting opportunity has arisen for a Band 6 Nurse to work within our highly skilled integrated multidisciplinary team in the Early Intervention Prevention Hub (EIPH).
The role of the Assessment Nurse in the EIPH is to gather extra information either by telephone contact or visiting a persons home, to identify clinical nursing or therapeutic needs in order to pass the person to the most appropriate care pathway.
The Assessment Nurse will work as part of a highly skilled multi-disciplinary EIPH team, working together to manage a cohort of referred people, all in need of various services.The Assessment Nurse will liaise and coordinate with colleagues across services to ensure the person receives the correct support.
The Assessment Nurse will have strong links with the Acute Clinical Team andhave the opportunity to work within this team, assessing patients, implement and evaluation to provide nursing support.

Main duties of the job
-
Provide effective listening & communication in the management of situations where there may be barriers to understanding. This may include sensory, communication difficulties, confused people, or where the atmosphere is highly emotive and potentially distressing.
Demonstrate excellent communication and negotiation skills in situations which may be highly sensitive and emotive. All communication will be underpinned by the principles of data protection, dignity and confidentiality.
Exercise leadership and develop collaborative relationships with multi-disciplinary team colleagues.
Ensure appropriate supervision and support of junior members of the EIPH team.
Exercise autonomy in clinical decisions, assessing patients' needs.
Ensure autonomy in managing own workload.
Actively function as a member of the nursing and wider multi-disciplinary team.
Ensure the security of patient records and equipment.
Ensure care planning is undertaken collaboration with the person and where appropriate with family/advocate.
Assist in the assessment, planning, implementation and evaluation of individualised and achievable programmes of care.
Assist with nursing and clinical duties
Advise, encourage and motivate patient's and their families to develop strategies to manage their own health.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
