Job summary

This is an exciting opportunity for an exercise practitioner to join our Long Term Condition Management services and work directly with Pulmonary Rehabilitation and Cardiac Rehabilitation services. The post holder will be supported with their clinical and professional development through working closely with Physiotherapists within the multi-disciplinary teams. This is a unique opportunity to use your professional exercise knowledge to work closely with physiotherapists and be part of established rehabilitation teams. The post holder will be developing and providing exercise programs for patients within the cardiac and pulmonary rehabilitation services. The work will involve group exercise & education in the community, as well as allocated 1:1 exercise provision. These aspects will be delivered face to face and digitally. The integration of exercise practitioners within these services is key to enhance the rehabilitation offer for our CTMUHB population, and ensures the best rehabilitation outcomes for patients with long term conditions.

The post holder will be expected to be proactive and enthusiastic, taking part in in-service training, education and research opportunities within services as directed by the clinical lead. We support flexible working.

Main duties of the job

Within this post you would be expected to work as an autonomous practitioner within a specialist Pulmonary / Cardiac rehabilitation teams, managing patients with specific conditions and be responsible for providing exercise assessment and treatment plans for patients. The post holder will work in partnership with members of the Pulmonary / Cardiac team, and any other relevant health professionals in providing expert advice to meet patient needs.

You would need to ensure patients' physical activities are developed and sustained in accordance with national guidelines and both national and local health and safety standards.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • Relevant undergraduate Degree-level qualification e.g. BSc Sports and Exercise Science (or equivalent).
  • Up to date Specialist Exercise Instructor qualification e.g. BACPR Level 4 Cardiac, Level 4 Pulmonary Rehabilitation, Level 4 Cancer Rehabilitation or equivalent, Level 3 personal trainer qualification.
  • Registration with an appropriate professional body i.e. BASES, CIMSPA.
  • Understanding of Rules of Conduct and the legal responsibilities
  • Aware of current evidence base and national guidelines for exercise within specialist area e.g. cardiovascular disease and/or heart failure, lung disease
  • Evidence of CPD sufficient to meet professional registration
  • Awareness of health and safety and clinical risk management
