Job summary

An opportunity has arisen for an experienced, enthusiastic and highly motivated Band 3 HCSW to join our team on Ward 7 based at the Ysbyty Cwm Cynon Hospital. We are a 15 bedded dementia unit providing person centred care and assessment for older adults with a diagnosis of dementia with significant mental health difficulties and associated challenging presentations.

The successful candidate will be expected to work collaboratively with the qualified staff to ensure that a high standard of care is maintained across the unit, and be expected to demonstrate a genuine interest in this field of nursing, displaying excellent people skills such as communication. They will be expected to work as part of a team, ensuring the delivery of high quality person centred patient care.Staff working within this environment are afforded the opportunity to experience diverse mental illnesses.

Main duties of the job

Successful candidates will be expected to hold an NVQ level 3 or equivalent or equivalent competency assessed training.

The post holder will deliver specific components of the patients care plan as delegated by the registered nurse and will regularly be required to work without direct supervision

Provide support to patients on an individual and on a group basis within the inpatient service

Organise and facilitate therapeutic activities such as group work along with registered staff or as directed by registered staff.

Maintain accurate and timely records of all contact with patients/ carers/relatives and other clinical contacts.

Ensure effective communication with all members of the Multi Disciplinary Team, advocates, and

Ensure all incidents which may compromise Health and Safety are reported in an appropriate and timely manner.
