Job summary

An exciting opportunity has arisen for a highly motivated and enthusiastic Band 5 Project Co-ordinator to support Cwm Taf Morgannwg University Health Board in the implementation of the Health Pathways programme.

The Health Pathways programme is the development and implementation of a pathways interface system which will provide primary care with evidence based clinical pathways for clinicians to support patients and services users to self-manage or receive care as close to home as possible to avoid referral to hospital for planned or unplanned conditions.

The Project Coordinator role is critical to providing comprehensive and effective support to the Programme Manager and Clinical Editors, contributing their organisational and communication skills to the development and maintenance of the system and pathway development.

Main duties of the job

We are looking for someone who is enthusiastic with excellent communication skills and able to build good working relationships and engage with all stakeholders across primary, community and secondary care on the developments, benefits and operational use of the pathways interface system.

The post holder will:

  • Liaise between the local Health Pathways programme and the Streamliners Writing Team in New Zealand.
  • Support clinical leaders, clinical editors and the programme manager with the delivery of the implementation plan.
  • Co-ordinate the development and maintenance of Health Pathways content and data

This post is fixed term/secondment until the end of March 2026 to meet the needs of the service. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.
