BAND 7 - FRAILTY & CHRONIC CONDITIONS NURSE - PRIMARY CARE CLUSTER
An exciting opportunity has arisen to support Primary Care Clusters to provide holistic care for their elderly, frail patients at their home including residential and nursing homes. This role will work closely with the GP practices, the Community Health & Wellbeing Team, social care and other services to provide community support for those with complex health and social care needs. This is an exciting nursing role, which will act as a dedicated link for those needing support to stay well and safe at home.
The successful applicant will be required to work initially across Taff Ely Cluster area with opportunities to develop into other areas, the role will support the health board in development of a Frailty Service based on evidence and population need.

- Provide pro-active and holistic nurse-led care whilst taking into account the complexities, sensitive situations and patient/family concerns and wishes.
- Develop pathways to support other primary care service providers
- Provide high quality care and advice to patients.
Main duties of the job
The post holder is expected :
- To case manage an identified group of people with long term conditions/complex needs, at a high risk of unplanned hospital admission with the aim of reducing unplanned admissions and improving health outcomes for that group.
- To support the prevention of unplanned hospital admissions, responding to patients who require urgent assessments and in collaboration with the patient, General Practitioner (GP) and Primary Care and community teams/social services, set up management plans to enable the patient to stay at home where possible.
- To advocate for high quality, consistent care Programmes for older people.
- To assess individuals, agree management/treatment plans and co-ordinate interventions within the multi-agency resources available with the aim of stabilising the individual's condition.
- For case managed patients, to support the transition from in-patient/residential care settings to home.
- To inform, teach and mentor clients and their families with the aim of improving the patient's ability to understand and manage their long term conditions.
- To collaborate with colleagues within the NHS and partner organisations to develop a consistent local multi-agency approach to managing long term conditions.
- To work in partnership with the Community Health and Wellbeing Team across the cluster area.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Essential
- Registered Nurse
- 1st level Degree or equivalent skills, knowledge and experience
- Evidence of continuing professional development in a relevant area of specialist practice to Master's equivalent level.
- Teaching and assessing qualification or equivalent demonstrable experience
- Non-medical prescriber/supplementary prescribing (or prepared to undertake training within 12 months of commencing role).
Desirable
- Community Nursing Qualification/Advanced Nurse Practitioner Qualification.
- Trained to interpret microbiology and pathology results.
- Knowledge relating to long term conditions.
Experience
Essential
- Experience of working as an autonomous practitioner.
- Case management experience.
- Working with adults with long term conditions.
- Good understanding of working in partnership with statutory and non statutory agencies to support patient centred care.
- Proven extensive experience of working with adults/older people

- Experience of working between other health and social care organisations/ settings.
