Job summary
An exciting opportunity has arisen for an experienced Mental Health Nurse to join the Older Adults Community Mental Health Team based in The Health and Well-Being Centre, Ysbyty George Thomas. The team provides secondary care support to those over the age of 65 years with severe enduring mental health conditions and works as part of a multi-disciplinary/multi agency service. We provide Care and Treatment planning as part of the CTP process and provide on-going assessment, support and monitoring to those living within the Rhondda area.The team operates 9am-5pm Monday -Friday.
The ideal candidate will have prior experience of caring for older adults in a health care setting, in particular, those living with Dementia or severe enduring mental health conditions.The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
To deliver evidence based holistic care to clients within the context of a multi-disciplinary, multi-agency community mental health service.
To hold a clinical caseload providing comprehensive assessment, care and treatment to clients referred to the Community Mental Health Team in accordance with Part 2 of the Mental Health (Wales) Measure 2010
Promote a positive working culture, working collaboratively with service users ensuring that their views and ideas are heard and equal
Foster robust, open and positive working relationships with all partners in particular Local Authority and other mental health services and the 3rd sector, ensuring that all partners are treated with dignity and respect
Proactively contribute to the continuing improvement and development of community mental health services, within an environment that welcomes new ideas and listens to everyone
Be guided by broad occupational policies.
Frequently work as a lone worker.
