Job summary
WISE Wellness Coaches are improving the health and wellbeing of everyone in Cwm Taf Morgannwg. Wellness Coaches are health professionals with a diverse range of skills who provide people with the knowledge, skills and confidence to become more active in their care to maximise personal well-being and overall health. Our Wellness Coaches use a range of evidence-based interventions and strategies to holistically support people's needs and for them to actively and safely engage in change.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
This post is Fixed term to 31 March 2024 to meet the needs of service.
Main duties of the job
The post holder will work independently in a variety of Community and Primary Care settings in CTMUHB, as part of the Wellness Improvement team. They will roll out the development and implementation of the Health Trainer Support across the Health Board area in a using a variety of methods including education sessions and individual person support to activate individuals to improve their health and wellbeing.
The post holder will be expected to undertake appropriate specified training to use the licenced software and training programmes that they will deliver. They will have excellent communication and behaviour change skills used to skilled conversations and brief interventions. They will have opportunities which will enable them to support and motivate patients to make positive lifestyle changes, identifying their own risk factors, set goals and enable them to reduce and manage their risk and condition.

- Level 3 Diploma in Clinical Healthcare Support, Fitness Instructor or equivalent qualification or experience
