You will provide highly specialist intervention to urgent swallow/communication referrals, offering domiciliary, outpatient, care home and virtual appointments jointly with our dietetic colleagues- predominantly across the South area of the UHB- covering RCT and Bridgend.
Main duties of the job
As well as forming an essential part of an AHP community network of clinicians across the UHB, you will also act as a key member of Cwm Taf Morgannwg's growing outpatient and community service, serving predominantly the Rhondda and Bridgend population in their own homes, aiming for joint assessment with dietetics wherever possible.
You will liaise closely with the supportive and collaborative AHP community and outpatient team, as well as working closely with the array of community services including 'At Home', 'Stay Well At Home', Reablement, CRT etc.
You will have access to regular supervision and opportunities for development. You will also have direct referral access to our VFS and FEES services.
