Job summary
The post is a new temporary post, and the successful applicant will join a robust General Medical team with aim of establishing a Care of the Elderly Services in Merthyr and Cynon ILG Cwm Cynon Hospital.
The post will be subject to the terms and conditions of service governing the appointment of Medical and Dental Staff in Wales, and to the Health Board's terms and conditions as appropriate.
The post holder will be part of a dynamic, forward looking and innovative General Medicine team. The post holder will be based at Ysbyty Cwm Cynon but be part of the wider General Medicine team based in Prince Charles Hospital inked Merthyr Tydfil, where the Health Board has acute hospital beds covering all medical and surgical specialities.
The post holder will be expected to help develop Care of the Elderly services provided by the Health Board. Wide experience in General Medicine and Care of the Elderly is essential. An interest in a Geriatric Medicine sub-speciality can be accommodated.
Main duties of the job
It's expected the post holder to undertake the following responsibilities: -
- To be involved in rehabilitation and discharge planning for frail elderly patients.
- To provide holistic care for elderly patients.
- To provide advice and support to medical teams dealing with patients with complicated Rehabilitation/Discharge Planning issues
- Lead on delivery of high-quality care in our communitybed facilities to serve increasing and more enhanced "step up" models of care, as well as rehabilitation.
- Specialist community MDT working (virtual ward models for planning care of complex patients) in partnership with Social Care, Mental Health, Primary Care, Specialist Community Nursing, Therapy, End of Life and emergency services.
- The post holder will augment the ongoing education and training of medical and allied health professionals in Frailty and the management of acute frailty syndrome presentations
