Job summary

The post holder will work within the multi-disciplinary Outreach Team, to deliver a high standard of patient care to the identified Outreach patients. The aim of the Outreach Team is to:

  • To avert admissions to Level 2/3 care.
  • To facilitate discharge from Critical Care.
  • To share Critical Care skills.

Main duties of the job

The post holder will work within the Critical Care Outreach team to ensure quality of care. The main aims of this position will be to enhance the standards of care for critically ill patients; irrespective of where they are being nursed in the University Health Board.

The post holder will be expected to complete the MSc in Advanced Clinical practice & Independent Prescribing. The post holder will work at a band 6 whilst training and progress to a Band 7 on completion of the MSc in Advanced Clinical Practice with Independent Prescribing.

The post holder will be required to ensure the highest standard of patient care, which reflects the quality standards and mission of the University Health Board and contribute to the implementation of policies and procedures within their sphere of responsibility.

The post holder will plan implement and evaluate care according to changing needs and varying complexity of the patient.

The post holder will prioritize and deliver care in collaboration with other health care professionals.

The post holder will involve patients and where appropriate relatives and carers in planning and delivering care.

The post holder will facilitate information exchange of patient related conditions and treatment at the end of the shift.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
