Job summary
The Mental Health Practitioner will be based within the GP practices across the Taff Ely area, offering both face to face and telephone appointments.
Each consultation is a 30 minute slot, unless otherwise required, consisting of a 20 minute consultation with the Mental Health Practitioner (MHP) and then 10 minutes to summarize and appointments and make onward referrals/discussions. Follow up appointments can be arranged if required.
Each MHP is highly skilled, with significant knowledge and experience within mental health services across Cwm Taf Morgannwg Health Board (CTM). Each MHP has a degree in mental health nursing or equivalent and are employed by CTM health board. Each Practitioner within the team are registered with the Nursing and Midwifery (NMC) Council or equivalent.
Each MHP can offer advice/knowledge regarding mental health issues and mental health services within CTM to all professionals within the GP practice.

The successful applicant will be required to provide services to a number of GP practices within Taff Ely

The role will involve:-o The post-holder will provide high quality care and advice to patients with mental health problems as well as take decisions to signpost individuals onto appropriate community service support.
- The post holder will work as part of a multidisciplinary Enhanced Cluster Team within a community setting to support GP's and other practice staff.
- The post holder will liaise closely with Third sector organisation, Local Primary Mental Health Support Services (LPMHSS) and Secondary Care Mental Health Services including crisis teams and to ensure seamless and purposeful pathways of care.
- There will be an expectation to produce evidence that can be used to inform future service design and commissioning of services. This will include using outcome measures, and promoting the highest possible utilisation of the service.
