Job summary
An opportunity has arisen for a keen, enthusiastic nurse to work as part of Bridgend South CMHT. The post is at band 6 and the successful applicant will work with a multi-disciplinary team.
You will be care coordinator for a caseload of patients and work within the framework of the Mental Health Measures (2010) Wales providing specialist knowledge and advice to a range of service users and providers.
Main duties of the job
As indicated, you will be responsible for a caseload of patients with varying mental health diagnosis. This will involve a comprehensive assessment of need, care planning, implementation of that care and regular reviews in line with team and national standards.
As a member of the team, there is a requirement to work weekends on a rota basis. This is shared between staff in both north and south CMHT's and generally staff are required to work approximately 4 weekends per year 9-5 including some Bank Holidays.
The successful candidate will be expected to work as part of the wider MDT but also autonomously. You will undertake assessments and be involved in some joint working.
