Job summary

The Specialist Dementia Intervention Team is a forward thinking, dynamic service committed to working in the field of Dementia care. The role of the team is to support carers of those experiencing significant behaviours that challenge associated with dementia. The team is based in Ysbyty Cwm Cynon and works in the communities and Care homes in Rhondda Cynon Taf and Merthyr. We are a multi disciplinary team consisting of Specialist Dementia Practitioners, Psychology, Occupational Therapists, Associate Practitioners and Health Care Support Workers

We are looking for a Registered professional (Nurse, Social Worker, Occupational Therapist, Paramedic etc.) who will work as a generic clinician to develop a psycho social formulation using The Newcastle Model to support carers of those living with dementia and experiencing behaviours that challenge.

The team operates a 7 day service with core hours of 9am - 5pm, however the service has the flexibility to cover a 12 hour day, the post holder will be required to work a flexible 8 hour shift pattern

Main duties of the job

The Specialist Dementia Practitioner will be expected to work generically whilst providing support to the Multidisciplinary team within the remit of their specialism. The practitioner will work closely with the Healthcare support workers to provide our clients the opportunity to continue living in their current environments and reduce the need for hospital admission. With the use and role modelling of behavior management techniques the team supports the care giver to de escalate and adapt approaches thereby reducing care giver distress.

You will be expected to possess excellent organisational and timemanagement skills as well as sound clinical assessment andformulation skills. As part of our assessment process you will be completing the Neuro psychiatric Inventory and the Bradford Wellbeing Profile as out come measures to support the effectiveness of our interventions.

The post holder will need to be resilient and self motivated withdrive and vision.Have the ablility to work on their own initiative and manage a range of priorities across competing agendas.They must possess the ability to display care, compassion and understanding to the client group.

You will be expected to support to the team manager with supervision of staff as the need arises and to display leadership qualities in their absence.

As part of the role you will be expected to participate in ongoing training, multi disciplinary events and audit/research.
