Health Care Support Worker – Frailty Support Worker

Job summary

The post offers an excellent opportunity to develop your skills and experience in caring for older adults with frailty needs. The focus of the role is to enhance the patient experience by promoting independence and supporting patients to maintain and maximise their functional ability.

You will be expected to encourage and assist patients with mobility, nutrition, and hydration, as well as support them to participate in daily interactive activities that contribute to their overall well-being.

The post holder should hold an NVQ Level 3 in Health (or equivalent). They must demonstrate effective communication skills, be an excellent team player, and also be able to work independently when required. You will work closely alongside both the nursing team and the therapy team to deliver high-quality, patient-centred care.

The ability to speak Welsh is desirable for this post; however, both Welsh and English speakers are welcome to apply.

Main duties of the job

Support the delivery of high-quality care to older adults with frailty needs, focusing on improving the overall patient experience. Promote and maintain patient independence, encouraging participation in mobility, nutrition, hydration, and daily interactive activities. Assist patients with personal care, including washing, dressing, toileting, and maintaining dignity at all times. Support safe mobilisation using appropriate equipment and follow moving and handling guidelines. Monitor and report changes in patients' condition promptly to the registered nurse or therapy staff. Carry out delegated clinical tasks, such as observations, fluid/food monitoring, and documentation as required. Work collaboratively with the nursing team, therapies team, and wider MDT to achieve individualised patient goals. Interviews to be held on 24/4/2026
