We're offering an exciting opportunity to join our evolving Pulmonary Rehabilitation and Cancer Therapy services at Hywel Dda UHB. This full-time physiotherapy role spans Pembrokeshire, Ceredigion, and Carmarthenshire, based at Glangwili Hospital with potential for virtual and cross-site delivery.
Our Pulmonary Rehabilitation service includes:
- Community-based exercise classes
- Education sessions
- Patient assessments
- Home visits
We work with the National Exercise Referral Scheme and are building partnerships with third-sector activity providers, to support patients beyond their rehabilitation programme.
This role also supports our established, digitally-led Cancer Therapy team. You'll help expand our "Prehab through to Rehab" programme, supporting patients:
- Before cancer treatment (prehabilitation)
- During treatment
- After treatment (rehabilitation)
The programme improves clinical, functional, and wellbeing outcomes, enhancing patient experience.
We're seeking a dynamic, experienced, and inspirational physiotherapist to join our multidisciplinary team and help deliver innovative, virtual cancer therapy services.
Main duties of the job
You'll be part of a friendly, proactive therapy team working collaboratively across community-based MDTs. This forward-thinking service supports professional growth and innovation. You'll deliver high-quality, evidence-based, person-centred care across both services, contributing to rehabilitation development across Hywel Dda UHB.
The post holder will form part of an existing team of physiotherapists responsible for the delivery of patient rehabilitation services within Hywel Dda University Health Board within their defined clinical area of clinical speciality. This will involve working in collaboration with inter and multi disciplinary teams (MDT) to deliver holistic care.
The post holder will be responsible for managing their own caseload which will involve self referrals as well as those from GP's, AHP's, Social Service practitioners and Consultants. They will operate as an autonomous professional with a high level of clinical and technical skill and expertise, using evidence base to plan, implement and evaluate the management of clients.
The post holder will participate in the continuous professional development process, involving the education / supervising of junior staff and students.
The post holder will play a key role in supporting service modernisation and development. This will include involvement in audit and service evaluation processes.
