This is a fixed term/secondment post until 31/03/2026.

This is an exciting opportunity for registered clinicians to work in Primary Care and Out of Hours undertaking home visits for palliative care patients, linking to provision of First Contact Practitioners (FCPs), managing patients at home and preventing admissions.

The primary focus of the role is to work as an Urgent Care Clinical Practitioner providing patient-centred clinical care, encompassing the skills of assessment, examination, diagnosis and treatment of patients. The role is for someone with a particular interest in palliative care and frailty, both in and out of hours and is intended to provide improved outcomes for patients and their families by improving timeliness and consistency of care.

Main duties of the job

The post holder will be expected to work as part of a multidisciplinary team but will also be expected to exercise a high degree of personal and professional autonomy and make critical judgments to satisfy the expectations and demands of the role. They will manage their own workload and work across professional, organisational, agency and system boundaries to improve services and develop practice. They will participate in networks, locally, regionally and nationally. The post holder will assess and manage risk and proactively challenge others about risk.

The post holder will be expected to work across Primary Care and Out of Hours on a rotational basis or to cover staffing as requested within their scope of practice.

Autonomously deliver a caseload of patients with palliative care needs, using evidence-based, patient centred principles to critically analyse, assess, plan, implement and evaluate interventions. This will include education and support on all aspects of the patient's care management plan.

Work collaboratively with other members of the multi-disciplinary team to ensure the health care needs of patients are met. This will include continual evaluation of the service and reflection on own practice.

  • Level 1 Registered Paramedic, Registered Nurse (Adult)
  • Current NMC / HCPC Registration
  • CPD to Master's level or equivalent experience
  • Master's level (level 7) qualification in Minor injuries/ illness or can demonstrate equivalent knowledge and experience at this level
  • ALS/ILS/PILS (Advanced Life Support /Intermediate Life Support/Paediatric Intermediate Life Support
