This is an excellent and exciting 6 month secondment opportunity to specialise your skills in Tissue Viability in the community setting. We are looking for an experienced individual in the field of wound care who would like to specialise in the community setting in Llanelli.
The post holder will work as an integral member of the Community Nursing Team, providing an expert professional and clinical contribution to the nursing care and management of patients with complex wounds in the community setting.
The post holder will facilitate relevant practice development in line with National standards, guidance and best practice through clinical practice, leadership, education, research and audit which reflect the cooperate clinical governance objectives.
To work as an integral member of the wider multi-disciplinary team to promote cost and clinically effective wound care.
The post requires excellent communication skills, a flexible in approach to work and good time management skills. Good IT skills are essential.
The post holder will need the ability to travel between sites in a timely manner.
Main duties of the job
The post holder will work closely with Medicines Management and Tissue Viability Team with regards to educating staff in the community care sector, promoting concordance with HDHB dressings formulary.
Work as part of the Community Leg Ulcer Clinic and promoting the Leg Ulcer/Wet Leg Pathway within the community setting.
Support the Community Nursing Teams in Service caring for patients with wounds in the community setting.
To use expertise and clinical skills to assess patient's individual needs, to educate and give practical advice on how to manage their treatments.
To recommend wound dressings and advise on treatments as per nurse prescribing legislation and Health Board protocols.
Work towards developing innovative and modern proposals for meeting the care needs of this client group.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
