Job summary

Hywel Dda University Health Board is seeking to recruit twoenthusiastic, energetic and passionate Smoking and Wellbeing Practitioners to develop and deliver support in primary, community and secondary care settings across the board.

The post holders will use a health coach approach to behaviour change to provide support and coaching sessions helping people to quit smoking, also looking at other lifestyle factors such as alcohol use, physical activity and nutrition.

A key focus will be on the development and expansion of the Health Coach clinics within primary care settings, seeking to build professional relationships with key members and developing a rapport with patients to support healthy lifestyle behaviours using evidence based behaviour change methods.

The post holders will work closely with the Smoking & Wellbeing team leads in particular the lead for health coach, as well as primary care partners to develop work in this crucial area and as such will be required to travel to various GP practices across the health board within their working week.

Main duties of the job

This is an exciting opportunity to work as part of a dynamic and busy team focused on improving the health and wellbeing of the population of Hywel Dda University Health Board covering Carmarthenshire, Ceredigion and Pembrokeshire. Knowledge and experience of utilising a Health Coach approach would be beneficial. Full health coach training will be provided.

The post holder will work as part of a growing, dynamic and busy team focused on improving the health and wellbeing of the population, strategic planning and partnership development within the wider Public Health Directorate of Hywel Dda University Health Board.

The team is focused on leading the Health Improvement agenda for the Health Board. This includes smoke free, health coach, smoking cessation and drug and alcohol misuse as part of a wider healthy lifestyle and wellbeing approach. The post holder will play a key role in delivery of the Health Coach Service including liaison with Primary Care.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • Recognised degree level qualification in Nursing, Health, Education, Psychology, Addiction or related areas relevant to the subject e.g. RGN, Diploma in Counselling, CQSW, Behavioural change qualification
