Job summary
An exciting opportunity has arisen for a Band 7 Heart Failure in-reach CNS to join the Heart Failure team in Pembrokeshire. We are looking for an experienced registered nurse with a high level of skill and theoretical knowledge. The post is 15 hours part time and will be primarily based in Withybush Hospital.
You will work closely with cardiology consultants, SDEC (Same Day Emergency Care), multi-professional cardiology colleagues, primary and community based nursing teams/services to reduce unplanned admissions to hospital.
Excellent verbal/written communication, organisational skills are essential for this role. A working knowledge of heart failure management and completion of a post-registration heart failure course is essential, whilst a non-medical prescribing V300 qualification would be desirable.
Main duties of the job
- Work with clients with highly complex needs using evidence based/client centred principle, and to assess, plan, implement and evaluate interventions across the primary/ community/secondary care interface.
- Assess patients, plan, implement and evaluate care, provide specialist advice and maintain associated health records and audit data.
- Work collaboratively with members of the multidisciplinary team to ensure that the health care needs of individuals are met, to include collegial heart failure nurses, General Practitioners, Consultants and Cardiologist, and allied community nursing services.
- Work autonomously in clinical premises e.g., hospitals, community clinics and possibly patients' homes to support the care needs of patients.
- Possess excellent communication skills and support and encourage patient/carer interventions and education; provide expert support, advice and act as an educational resource to colleagues as required.
- Demonstrate Health Board values and promote those values in others.
