An exciting opportunity has arisen for a Band 6 Heart Failure Clinical Nurse Specialist to join the Community Heart Failure Team in Pembrokeshire. We are looking for an experienced registered nurse with a high level of skill and theoretical knowledge.
The post is 15 hours part time (over two days).
The CNS team provide specialist nursing knowledge and skills for people diagnosed with heart failure. They provide multidisciplinary care planning and care interventions designed to reduce hospital admissions, with the ultimate aim of improving patient outcomes. The team work closely with cardiology consultants, multi-professional cardiology colleagues, primary care, and community based nursing teams/services.
Excellent verbal/written communication, organisational skills are essential for this role. A working knowledge of heart failure management and/or completion of a post-registration heart failure course is desirable.

Main duties of the job
The CNS will efficiently manage a defined caseload of clients with highly complex needs using evidence based/client centred principles, and to assess, plan, implement and evaluate interventions across the primary /secondary care interface.
The CNS will deliver educations to patients, families and colleagues to promote excellence in heart failure care.
The CNS will holistically assess patients, plan and implement care, provide specialist advice, review care plans/interventions and adapt as necessary. Maintain accurate nursing records for all interventions and clinical encounters.
The CNS will work collaboratively with other members of the multidisciplinary team to ensure that the health care needs of individuals are met, this includes in-reach heart failure CNS, General Practitioners, Consultants and Cardiologist, and allied community nursing and health services.
They will work autonomously in patients' homes, clinics and hospitals to co-ordinate the care needs of patients.
The CNS will actively seek out learning to advance and uphold their knowledge and skills in heart failure nursing. The successful candidate will be expected to undertake post registration training e.g. Glasgow Caledonian Chronic Heart Failure: Optimising Health & Wellbeing level 7.
