Main duties of the job
As one of our Community Integrated Support Workers/ Homecarers, your duties will include working side-by-side with Occupational Therapists, Physiotherapists, Nurses GP's Dieticians and other health and social care professionals, undertaking delegated tasks and providing planned care and support to people that will help them to remain in their own homes.
Our Community Integrated Support Workers/ Homecarers will work closely with other agencies to help individuals reach their goals, for example, Delta, Red Cross, Acute Response Team, Mental Health Team, Substance Misuse Teams and Social Workers.
The ability to speak and write in Welsh is essential for this post.
