This is an exciting prospect for GPs to contribute to Hywel Dda University Health Board's Frailty team at Withybush Hospital.
This post offers scope to work alongside the Care of the Elderly Team to develop and enhance the frailty pathway.
Given the developmental nature of this opportunity, the post holder may be required to undertake duties which may vary to those outlined in the job description. The post holder will however be instrumental in guiding the development of the new model.
Main duties of the job
In accordance with the rota, the post-holder will undertake a variety of duties, in accordance with best practice, including assessment, diagnosis [including interpretations of radiographs and blood results as appropriate] and management [including wound management] of minor illness in a TIMELY manner.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers
Screening patients for disease risk factors and early signs of illness and Frailty.
Identifying patients who may benefit from the Frailty Clinic,
Work closely with the acute medical admissions team and manage patients according to agreed clinical pathways
In consultation with patients and in line with evidence-based best clinical practice developing treatment and any associated care plans.
Admitting or discharging patient to and from the caseload and referring to other care providers as appropriate.
Championing the service's clinical governance programme for the provision of safe, high quality patient care.
Working with the Senior Management Team on clinical governance issues and clinical aspects of the service.
Prescribing in accordance with local prescribing formulary [and generically] whenever this is clinically appropriate.
Ensuring that when referrals are made for patients to see other medical service providers, adequate information is given to the personnel to whom the patient has been referred for proper assessment to be made.
