Job summary

An opportunity has arisen for a bright, keen motivated Nurse with experience of working with people with Diabetes to join our team in delivering essential and specialist Diabetes nursing care within the acute setting.

The Diabetes Team provide in-patient and out-patient services for people with diabetes across Pembrokeshire and neighbouring counties. Our services include specialist diabetes care for patients with type 1 diabetes and those patients with complex type 2 diabetes, insulin pump therapy, support for the young adults service, structured patient education and staff education as well as providing an in-patient service for patients with diabetes.

Main duties of the job

We are looking for a colleague who enjoys team working and who has the ability to function as an autonomous practitioner.

You will be expected to liaise with colleagues in secondary and primary care, providing specialist advice to healthcare professionals, other agencies, patients, carers and relatives.

To utilise advanced nursing skills and extensive knowledge base to undertake specialist assessments, critically analysing complex information to make informed decisions regarding appropriate treatment/intervention to ensure the delivery of highly specialist care.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
