Job summary

We have an exciting opportunity in Hywel Dda University Health Board with this Community Team Lead vacancy based in the county of Pembrokeshire. This post will support Leadership and Clinical Service delivery for Community Teams in this region of the Health Board.

Recent changes have resulted in the amalgamation of the North and South Pembrokeshire Community Teams into a single, county-wide service. This Team provides visits to patients homes as well as Residential and Nursing homes, provision of balance classes, Pulmonary Rehabilitation, provision of Physiotherapy into four wards (two slow stream rehabilitation ward, one Acute Adult Psychiatric ward and one Older Adult Psychiatric Assessment ward).

Community physiotherapy is required to provide a range of Neurological, Respiratory, Frailty/T&O and MSK interventions for patients from a wide range of backgrounds who are unable to access services elsewhere. Referrals come from Inpatients services, GP's, Mental Health services, other professionals and Consultants among other services.

We are looking for an enthusiastic, motivated individual who has proven leadership qualities and significant experience of working with complex case loads.

The University Health Board supports post graduate training and other forms of internal/external CPD.

Main duties of the job

As Team Lead, you will manage the waiting list, manage the annual leave, sickness reporting and recruitment of staff within the Community setting. You will also continue to develop the criteria of prioritisation and work closely with the Clinical Leads within the Community setting in the provision of a high-quality, proactive and effective service.

The successful candidate will work collaboratively with the MDT across the Acute and Community pathway supporting delivery of effective clinical interventions and service development for the future.

This candidate should be confident in dealing with staff management as well as clinical work.
