Job summary
North Ceredigion Crisis Resolution and Home Treatment services are looking for a compassionate and motivated senior mental health practitioner to join our well established team in Aberystwyth. The team is multidisciplinary and mental health nurses, occupational therapists and social workers are encouraged to apply.
The role offers the opportunity to work in acute mental health care in the community, assessing and supporting people in acute mental distress. Working alongside a small team of senior practitioners and health care support workers, practitioners are encouraged to work holistically and creatively to support individuals to get past a period of crisis.
The role involves working with a wide range of services to help meet an individuals needs and is often fast-paced, requiring a rapid and responsive service.
The ideal candidate will have excellent risk assessment and management abilities and be focused on providing a compassionate human response whilst utilising strong therapeutic skills.
The service operates 24 hours a day, 7 days per week, receiving referrals for assessment from A&E, GPs and other local services. The team manages a small caseload of individuals in acute need of mental health support which is provided both at our base in the Community Mental Health Centre, Gorwelion (CMHC) and in a person's home.
Main duties of the job
Receiving referrals from GPs, A&E and other agencies, and screening for risk / appropriateness for assessment.
Assessing individuals in acute mental distress in A&E, in CMHC, and in their homes.
Assessing risk, particularly of significant self-harm or suicide, and developing and implementing risk management plans.
Working therapeutically and practically to provide bespoke support to individuals based on the presenting needs as assessed.
Developing and implementing plans for community treatment as an alternative to hospital for individuals in acute mental distress.
Working alongside and supervising Health Care Support Workers in delivering brief interventions and guided self-help.
Working with other mental health services, GPs, A&E, third-sector organisations, and agencies involved in meeting the many and varied social needs of individuals.
Supporting inpatient services by facilitating early discharge fr.om hospital with intensive home treatment where appropriate.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
