It is with excitement, that following recent investment and re-evaluation of our Physiotherapy service in Llanelli - the biggest town in Carmarthenshire, Hywel Dda University Health Board presents this vacancy, within the front of house MDT service.
There is a very positive, forward thinking and dynamic working environment present. We are therefore looking forward to introducing an enthusiastic and motivated individual who has proven clinical skills in the field of medicine, acute and community rehabilitation to join our team.
Main duties of the job
We are committing as a service to improve our acute-community pathway collaboratively with the MDT to bridge the gap between the primary and secondary settings. As well as joint working session with social workers and occupational therapists.
The successful candidate will be working across the acute and community settings to facilitate the integrated care pathway in Llanelli, to complete efficient and timely complex assessments as well as discharges.
Also to provide admission avoidance in the community and managing patients presenting with frailty syndrome. We already have strong links with the rehabilitation and geriatric consultants in Llanelli who are eager to work closer with us therapists to develop services.
