Diabetes Specialist Nurse

Job summary

We are looking to recruit a motivated individual to work full time and deliver a comprehensive, high quality evidenced based Diabetes Specialist Nursing service to people with diabetes in Mid Powys in collaboration with our small team of Specialist Nurses and the Multi-disciplinary Team across Primary, Community and Secondary Care.

The role covers in-patient, out-patient and community care, actively contributing to the planning and development of the Diabetes services across Powys. Providing an advanced level of practice that encompasses aspects of education, research and management.

Main duties of the job

You will be responsible and accountable for an identified case load of people with diabetes, proactively promoting health enhancing activities and behaviours, working with the patient / carer and the MDT team to reduce unplanned hospital admissions and co-ordinate care across all settings of the care pathway.Promote and ensure through ongoing review and monitoring that people with diabetes are treated in line with current guidelines and that the principles of dignity and respect are achieved and embedded within the team.

  • Live registration with the NMC as Registered Nurse
  • Degree in Nursing or health related subject
  • Post Registration specialist training / supplementary qualifications to Masters level in related subject (Diabetes Speciality
  • Teaching qualification/ Teaching and Assessing in Clinical Practice or evidence of teaching skills/ clinical educator
  • Knowledge and skills to manage patients with complex needs

Desirable

  • Independent Prescribing Qualification
  • MSc Advanced Clinical Practice
  • Management/ Leadership Qualification or equivalent preparation

Experience

Essential

  • Experience at management level demonstrating ability to deliver improvements to service users care
  • Experience of clinical assessment and proactive management of people with diabetes

Aptitude and Abilities

  • Ability to use computer systems to a well-developed level (email, word processing, spreadsheets)
  • Significant clinical experience in assessing, planning, implementing and evaluating care for the patient group of the caseload
  • Well-developed verbal and written communication skills
