Job summary

THIS POST IS FIXED TERM/SECONDMENT FOR 12 MONTHS DUE TO MATERNITY COVER.

The post holder will:-Work within an established, integrated, multidisciplinary team, focusingupon individuals with severe and enduring mental health problems.

Undertake responsibility for the care co-ordination and case managementof a defined caseload of clients with complex and challenging needs asdefined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.

Within their role as Care Co-ordinator will facilitate the promotion andmaintain a high standard of nursing care adhering to current legislation,professional and corporate guidelines.

Main duties of the job

You will be supported by a multidisciplinary team to provide specialist assessment, treatment and care to a caseload of people from within the local community who experience mental health difficulties.

The successful candidate will be required to work with service users within the legal frameworks of the Mental Health (Wales) Measure 2010, the Social Services and Well- Being (Wales) Act 2014 and the Mental Health Act (1983).

You must be enthusiastic, self-motivated, flexible in approach, and able to work autonomously with statutory and non- statutory service providers.

Effective communication and engagement skills are fundamental to this role, as well as current evidence-based knowledge, a commitment to working with individuals adopting a recovery approach and a commitment to working within a trauma informed model of care. We are keen to support practitioners to develop their skills and knowledge in therapeutic interventions and group work.
