The Powys Living Well Service is seeking a dedicated and compassionate Assistant Practitioner (Band 4) to play a key role in supporting individuals living with persistent pain, fatigue, and weight management. As part of our team, you will also contribute to service improvement projects, helping to shape and enhance the delivery of care to ensure the best possible outcomes.

Main duties of the job

As an Assistant Practitioner, you will independently manage your workload while working under the supervision of a Registered Practitioner to support individuals with long-term conditions. You will provide high-quality, evidence-based care, contribute to assessments, and support rehabilitation and self-management programmes.

Key Responsibilities:

  • Delivering Care

    • Support initial consultations, implement and modify care plans.
    • Assist in the delivery of rehabilitation sessions and weight management interventions.
    • Promote person-centred care to improve health and independence.
  • Monitoring Progress

    • Recognise and respond to changes in people's physical or mental conditions.
    • Provide accurate feedback and contribute to ongoing assessments.
    • Ensure safe and high-quality care, escalating concerns as needed.
  • Mentoring Junior Staff

    • Support and supervise healthcare assistants, and students.
    • Provide guidance and contribute to a positive learning environment.
    • Uphold professional standards and promote best practices in care delivery.
