Physiotherapy Rehabilitation Lead

Are you an experienced Physiotherapist looking for an exciting opportunity to shape and develop rehabilitation services in Powys?

We're looking for a motivated professional to lead the Physiotherapy rehabilitation provision within community services. This role offers the chance to help design and improve services across Powys, aligning with national guidance and key healthcare programs, such as the PTHB Urgent and Emergency Care, Frailty, and Community Model Programme.

What You'll Do:

  • Lead and support the training and development of Therapists and Therapy Support Workers delivering physiotherapy in the community and community hospital settings.
  • Help prevent avoidable hospital admissions, support falls prevention, and enhance unscheduled care pathways.
  • Hold a clinical caseload within Mid Powys while also developing services across Powys.

Main duties of the job

Have relevant knowledge and experience of frailty and be able to effectively use a variety of assessments and interventions.

Be a team player, with excellent communication skills and be committed to excellence in seeking the most robust and evidence-based models of intervention to enhance quality of life and occupational participation of those individuals living with frailty.

Demonstrate a high level of clinical judgement, working autonomously in patient's homes, care homes and primary care settings.

Contribute to areas of service development which will include attending meetings, audit and service evaluation.

Participate with the multi-disciplinary/agency team to provide a coordinated approach to management of people's care.

Involve service users, relatives, and carers in a persons management plan,including overcoming barriers to communication, gaining consent for assessment and interventions, and providing advice and education.

Influence the care provided by other Health and Social Care professionsthrough provision of specialist guidance and involvement in formal teaching. Take a proactive role in the clinical leadership and supervision of staff within specialist area. This may include supervising/mentoring MDT staff members and will include participating as a supervisee.

Participate in MDT training, both providing training and receiving training to develop own skills and those of the MDT in providing person centred prudent care.
