Job summary

An exciting opportunity has arisen for a Clinical Nurse Specialist to join the Specialist Palliative Care Team within Powys Teaching Health Board based in Llandrindod Wells.

The successful post holder will need to be flexible to meet the needs of the service, they will be based in Llandrindod Wells and will form part of the team providing specialist palliative care to the residents of Powys. The post holder will provide specialist palliative care to the residents of Powys.

We are a rural Health Board with 133,000 people living across an area that is a quarter of Wales. With no District General Hospital we provide as many services as possible locally through GP provision, Community Services, Community Hospitals and Clinics.

The Specialist Palliative Care Team comprises of Palliative Medicine Consultants, Clinical Nurse Specialists, Community Palliative Care Nurses and Secretaries. The team provides specialist palliative care within the community, supporting palliative patients within their own home, care homes and during admissions to our Community Hospitals.

Main duties of the job

To deliver a comprehensive, high quality evidenced based palliative carenursing service to a designated population within Powys, in collaboration with the multi-disciplinary Teams across Primary, Community and Secondary Care along integrated pathways of care.

Actively contribute to the planning and development of the palliative careservice across Powys.

As a team of palliative care specialist nurses provide training and development to Powys Teaching Health Board (PTHB) staff and partners.

Responsible and accountable for an identified case load, proactively promoting a person centred approach, working with the patient / carer and the MDT team to reduce unplanned hospital admissions and co-ordinate care across all settings of the care pathway.

Provide specialist expert clinical leadership in the field of palliative care toinspire colleagues to improve standards and practice e.g. Palliative Care MDTs and case conferences.Attend and participate in team meetings and represent PTHB when appropriate locally and nationally.

When required, facilitate, attend, chair, and deliver identified actions of staff meetings and ensure effective dissemination of information and best practice.

Work with and support GP clusters to review examples of end-of-life careprovision to identify opportunities to improve care.

Act as a line manager for other staff as appropriate.

Working with other Clinical Nurse Specialists and MDTs to ensure that allpatients who have specialist palliative care needs are supported appropriately.

Quality, Safety and EffectivenessEnsure that lines of communication are maintained with key workingrelationships to demonstrate politeness, courtesy, compassion and sensitivity, promoting a positive image of the health board.

Provide evidence-based information, knowledge, support and training onpalliative care treatment options and outcomes. Supporting patients in order that they can make informed choices.

Apply the principles of Being Open in communicating with patients families and appropriate others in all aspects relating to concerns.

Provide professional support and specialist clinical knowledge on palliative care to members of the community resource teams and work jointly with staff as necessary to promote best practice.

Support relevant public health and community development initiatives.

Undertake patient satisfaction surveys/patient stories at regular intervals and contribute to interpreting the results and action plans with the Senior Nurse.

Support the identification, investigation and implementation of learning from concerns within the Health Board. This may necessitate a lead role as defined by the Head of Nursing and require investigation and report writing.

Consistently work with teams to ensure standards of patient care are high and maintain safety and quality of the patient experience.

Support the effective transfer of patient care to and from your case load byensuring that appropriate assessment, communication and liaison thusfacilitating safe admission and discharge. Work specifically with Carers toensure an equal partner approach to ongoing care.Act as a responsible Nurse Prescriber, where appropriate, to prescribing in line with the Prescribing Formulary, Health Board/ All Wales/ NMC policies/Royal Pharmaceutical Society Competency Framework and guidance and making safe, clinically effective and cost-effective prescribing decisions.

Provide specialist assessment of patients, through clinics and home visitsundertaking a range of clinical measurements and assessments, interpret the results to inform treatment approach.

Review the effectiveness of treatments through monitoring and interpretation of clinical indicators and make adjustments to treatments, onward referrals and/or recommendations for changes to treatment or appropriate therapies as appropriate.

As a team review the service operational policy and work plan, contribute to the Powys end of life plan and report and support the development of co-ordinated clinical policies in all relevant areas with expert evidence-based advice and information.

Take a key leadership role in implementing quality improvements into the area of responsibility.

Take a zero-tolerance approach to practice which does not meet the highest standard working with staff/teams to consistently improve patient outcomes and experience.

Develop clinically effective and therapeutic working relationships with patients and relatives/carers using negotiating and motivating skills to communicate complex, contentious and sensitive information.

Through bereavement support and through the MDT identify where complex grief patterns are present facilitating referral to specialist bereavement support services.
