Job summary

The Early Intervention in Psychosis (EIP) service is focused on the early detection of and recovery from psychosis. Our service will work with clients aged 18-35 initially, with a good working relationship and robust transition arrangements in place with Child and Adolescent Mental Health Services.

The post holder will work as a member of the Early Intervention Psychosis team, under the direction and supervision of the Team Leader and the Mental Health Practitioners within the team, to ensure that an efficient and effective service is delivered to Powys residents.

The role of the Assistant Practitioner is to work 1:1 or in groups with individuals who are or have experienced psychotic symptoms. This will include the provision of support to their families and carers.

Main duties of the job

The post holder will be required to undertake a range of clinical and psychosocial interventions such as - anxiety management, cognitive behavioural skills and family intervention, in order to promote mental wellbeing. These activities may be carried out in a variety of settings in the community.

The post holder will support the clinicians to set up and run the physical health monitoring clinic, which will include taking bloods as well as other physical monitoring of patients and keeping accurate record keeping.

To support the service users to achieve their identified goals as detailed in the care and treatment plan, under the direction of the qualified practitioners in the team. The post holder will be responsible for prioritising patient activities according to the patients' clinical need and/or geographical location. The post holder will work with autonomy when planning and organising the goals with service users.

To assist in the assessment process, by gathering information through your observations and time spent with clients, families and carers and feeding this information back to the multi-disciplinary team and care coordinators.

Job description

Job responsibilities

The post holder will be required to undertake a range of clinical and psychosocial interventions such as - anxiety management, cognitive behavioural skills and family intervention, in order to promote mental wellbeing. These activities may be carried out in a variety of settings including Patients Homes, Clinic Settings, GP Surgeries, Care Homes, Day Centres and/or Community Hospitals and around the local community.

The post holder will support the clinicians to set up and run the physical health monitoring clinic, which will include taking bloods as well as other physical monitoring of patients and keeping accurate record keeping.

To support the service users to achieve their identified goals as detailed in the care and treatment plan, under the direction of the qualified practitioners in the team. The post holder will be responsible for prioritising patient activities according to the patients clinical need and/or geographical location. The post holder will work with autonomy when planning and organising the goals with service users.

To assist in the assessment process, by gathering information through your observations and time spent with clients, families and carers and feeding this information back to the multi-disciplinary team and care coordinators.

To monitor and update any risk issues in relation to patient care and safety.

To maintain liaison between clients, families and carers and the multi diplomacy team.

To recognise changes in the clients mental and physical health condition and be able to convey this presentation to the qualified practitioner. The post holder will escalate concerns to the mental health practitioner, as soon as possible to enable prompt intervention and reassessment of their needs as appropriate.

To provide effective communication with other professionals, clients and carers in a range of mediums e.g. emails, electronic database etc.

To have a clear understanding and awareness of triggers for safeguarding issues and child protection issues, reporting such issues to the Mental Health practitioners.

To be creative in your approach to encourage engagement with the client and mental health services.

To provide supervision or support for band 3 Health Care Support Workers, under the direction of the qualified practitioner.

To support the facilitation of training & education sessions to families/carers and other professionals.

To ensure accurate records are kept of all lines of contacts in line with departmental guidelines.

To maintain accurate patient records, both written and electronic, in accordance with Health Board Policy having regard to information governance, GDPR, confidentiality and safekeeping.

Maintain own ESR record in a timely manner.

Participate in undertaking audits and surveys.

As delegated by the Team Leader and Mental Health Practitioners, source patients/relatives/carers views and feedback on their experience of care services in order to influence and inform future provision.

To attend meetings as required and ensure that information is fed back to the Team Lead.
