Job summary

An exciting opportunity has arisen within our Primary Mental Health Service provision for a Primary Care Mental Health Specialist Practitioner, this is a new post with scope for developing and improving the care pathways offered. The post-holder will be an extension of the primary care practice working alongside GPs in providing an augmented mental health service to individuals currently on GP caseloads who present with acute mental health issues or a deterioration in a medium/long term condition.

We are seeking an enthusiastic and self-motivated professional who can provide clinical expertise in mental health, advanced practice, and specialist knowledge in support of the delivery of access to secondary and primary mental health and psychological therapy services. Their expertise will support GPs in the local care and management of their patients and provide a link to other Mental Health support services in the area.

Main duties of the job

THIS POST IS FIXED TERM/SECONDMENT FOR 12 MONTHS DUE TO FUNDING

If you are an internal applicant this post will only be offered on a secondment basis and you will need to have gained your line managers approval for the secondment prior to going to interview

The successful applicant will work predominately within the General Practices based in Llanfyllin and Newtown, alongside all disciplines and professionals to help educate, develop, and support a clear understanding of the complexities and scope of mental health need within the local population. They will also consult closely with secondary care mental health services and third sector organisations to ensure cohesive and purposeful pathways of care. The post holder will be expected to take a lead in local audit, learning, training, and development within the role. There will be an expectation to produce evidence that can be used to inform future service design and commissioning of services. This will include using outcome measures, compiling reports, and presenting findings. The role promotes and provides early assessment, treatment and improved access to evidence-based interventions for people with SMI and will help people to focus on achievable goals and access local community resources.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and/or knowledge

Essential

  • Professional qualification (one or more of the following): o Registered Mental Nurse (Nursing & Midwifery Council Registered) o Registered Nurse for Learning Disabilities (Nursing & Midwifery Council registered) o Degree or Diploma in Social Work (General Social Care Council or Health and Care Professions Council or Care Council for Wales registered) o Degree or Diploma in Occupational Therapy (Health Professional Council registered)
  • Postgraduate Doctorate in Clinical Psychology (or its equivalent) as accredited by the British Psychological Society (Health Professional Council registered and eligible for Chartered Clinical Psychology status of the BPS)
  • Relevant Master's Degree or equivalent experience
