Job summary
We are looking for a Care Transfer Co-ordinator who is motivated, enthusiastic, individual to work in collaboration with the multidisciplinary teams in facilitating the seamless transfer of patients from nominated District General Hospitals to the patient's own home, residential home, nursing home or a community hospital.
The base for the job is The Royal Shrewsbury Hospital, and this will be the place of work most days, where you will have the opportunity to work alongside colleagues from Powys and Shropshire to ensure our patients receive the most appropriate care in the most suitable location.
The post holder must be a Registered Nurse and hold relevant qualifications with their professional body.

- Live registration with relevant professional body NMC or HCPC
- Degree in Nursing/ Health related subject or demonstrable professional academic development to degree level
- Knowledge of current issues within NHS care systems
- Significant Knowledge of NMC code of conduct and professional standards/ guidance
- Knowledge,, ability and skills to manage patients with complex needs
- Experience of clinical assessment and proactive caseload management of patients
- Significant clinical experience in assessing, planning, implementing and evaluating care for the patient group of caseload
