Job summary

The post holder will provide on request, assessments of Nursing Mental Health issues at the Community Hospitals and Care Home settings. The assessment will occur upon the ward requesting advice, within an agreed time scale.

Provide specialist advice to the wards which supports the development of a care plan, formulated to meet the needs of the patient.

Provide specialist mental health assessment, in-reach, and treatments advice of patient's aged 65 years and upward. Concessions are made for patients with organic illnesses that would be better managed by the Older Persons Mental Health Team.

The promotion of mental health improvement will be interfaced with theirphysical independence, thus encouraging a speedier return to their place ofdwelling, and avoidance of unhelpful lengthy admissions.

The post holder can also be seen as a link between the existing Mental Health Medical staff and Community Psychiatric Nursing already available.

This is an attempt to supplement existing support in its ability to assess, implement, plan, and evaluate care in Community Hospitals requiring specialist mental health advice.

In acute situations where the ward teams may be experiencing great difficulties in management of treatments, the post holder may be able to give advice on "The Code of Practice" (Mental Health Act 2007).

Main duties of the job

As a Mental Health Liaison Nurse in Old Age Psychiatry you will provide a high quality liaison service to people aged 65 and over, admitted to one of our 5 Community Hospitals in Brecon, Bronllys, Llandrindod Wells, Knighton and Ystradgynlais who are experiencing symptoms of functional or organic illness. The work is interesting and varied, and the liaison role is well supported by other members of the Older People's Team. Our liaison service operates from Monday to Friday, 9am to 5pm (excluding public holidays).

Good practice is encouraged by reflective individual supervision. You will be supported to develop existing skills in mental health assessment and planning, in delivering teaching to general nurse colleagues and in undertaking audits. There are opportunities for personal and professional development and training. You should be a qualified nurse, registered with the NMC and with post-qualification experience. Prior experience of liaison or Community Mental Health Team working would be a bonus. You will need to have good communication and interface skills, good timekeeping and organisational skills, teamwork and leadership qualities.
