Job summary

As a Dual Diagnosis Mental Health Practitioner, you will work within a proactive and positive multidisciplinary team, focusing upon individuals (including young people) with severe and enduring mental health problems who have co-occurring Substance Misuse issues.

You will have responsibility for the care co-ordination and management of a defined caseload of clients with complex and challenging needs, providing liaison between statutory and other services particularly with colleagues providing substance misuse, housing and homelessness support and working closely with social workers and disability teams (social services).

Main duties of the job

.

You will be an integral member of the Team, providing training and specialist advice as and when needed to other staff and undertaking interventions as defined within Parts 2 and 3 of the Mental Health (Wales) Measure 2010. Assisting the CMHT Team Manager with the management and organisation, including advice and support, of their clinical area associated with dual diagnosis will be an essential element of the role.
