Heart Failure Specialist Nurse

Job summary

To develop and sustain a service to improve the health and well-being of individuals and their carers who suffer from Heart Failure.

To be involved in the management of heart failure to both primary and secondary care

To be able to act autonomously in the assessment and management of Heart Failure patients using protocols, policies, clinical supervision and clinical judgement to guide practice.

Work in partnership with staff across BCUHB and Primary care to ensure patients with heart failure are managed appropriately and to act as a specialist resource to other health care professionals.

Provide education and training to healthcare professionals about heart failure, ensuring all information disseminated is evidence based, thus facilitating a rise in overall standards of care across BCUHB.

Main duties of the job

An opportunity has arisen for a motivated individual to join the North Wales Heart Failure Service based in West.

The successful post holder will be required to manage a caseload of complex heart failure patients as part of a team approach. The key aims are to keep patients well at home to avoid hospital admission where possible, but also to support those living with a debilitating condition and their families. The service is fully integrated with other service providers to ensure care is offered at the right level to meet patient needs and meet BCUHB commitment to a prudent approach to healthcare. You will be responsible for managing patients with complex heart failure mainly in a community setting. The team also input into secondary care to ensure case identification, referral and clinical management is delivered to a high standard. The team use a virtual telephone clinic for the assessment and management of patients alongside community clinics . You will be required to have advanced skills such as clinical history and examination + non-medical prescribing. You will also support the team in delivering training and education in primary, community and secondary care settings as well as to patients and carers.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
