Betsi Cadwaladr University Health Board in North Wales is a dynamic and patient focused organisation. We are looking for an enthusiastic and highly motivated Practice Nurse who believes in a vision for high quality clinical care and compassion within Primary Care, and who wants to engage in delivering this in Hillcrest Medical Centre, Wrexham.
The successful applicant should already have experience of long-term conditions and working in Primary Care or General Practice.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
The post holder will perform tasks prescribed and delegated by the GPs and Senior Nurses, working unsupervised to Patient Group Directions, agreed protocols, within an evidence base and within defined competencies.
The main duties of the role include (but are not limited to):
- Undertake chronic disease management clinics, including Asthma, COPD/Spirometry, Diabetes and Chronic Heart Disease.
- Identify, and manage as appropriate; treatment plans for patients at risk of developing a long-term condition.
- Perform basic nursing tasks such as ear syringing, blood pressure monitoring, ECGs etc.
- Assess and care for patients presenting with wounds including the removal of sutures and clips and leg ulcer management.
- Participate in vaccination and immunisation programmes; administering injections, ensuring safe vaccine storage and implementing cold chain policy.
- Undertake health screening, surveillance and new-patient screens.
- Take pathology samples from patients for analysis (blood, urine etc).
- Assess problems presented opportunistically by patients, dealing with minor illness as well as in minor illness clinics.
The role will require flexibility to meet the demands of this large practice and an individual work plan will be agreed with the successful applicant.
