Occupational Therapist (Band 6)

Job summary

The post will involve focus on admission avoidance and discharge facilitation to support theDischarge to Recover and Assess service. This will include assessing and treating patients within the acute hospital and/or in their own homes.

The successful candidates will join an established multidisciplinary team which provides evidence-based interventions in a timely manner. The team is based in Ysbyty GwyneddEmergency Department and works closely with the Specialist Doctors and Nurses within the Emergency Department, Frailty Unit and Medical Admissions Ward, as well as supporting patients out into the community when required.

The post isa part time (30 hours a week) permanent post. The service has extended hours and weekend working which any successful candidate would need to be available to support as per the team rota.

Main duties of the job

  • Confidently carry out comprehensive and professional assessment andinterventions for patients with complex needs incorporating physical, personal and psychological needs.
  • Based on strong Clinical reasoning, and positive risk taking, to support home first discharges in line with Welsh Government policy.
  • Ability to work within fast pace and unpredictable environment
  • To communicate with these patients, carers, agencies and other disciplines in an appropriate manner, often with sensitive information, diagnoses or outcomes.
  • Have excellent organisational and communication skills
  • Provide supervision and development of support workers and students within your team
  • Have the ability to use computer systems in order to update clinical information necessary for the job role including data collection and audits
  • Maintain and update own knowledge as required by the HCPC - active encouragement is promoted to seek internal and external training and development opportunities
  • The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
