We have an opportunity to join the Home First Team as a vital admin assistant. The successful post holder will manage the day to day data entries of patient trackers and assist the Clinical Flow team with escalations/queries/chasing.

This role is essential to the Clinical flow team to progress the discharge of patients within the hospital/community hospitals.

Full training will be provided to the post holder on the area of work and also the Home First Admin role in full.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

The post holder will support the delivery of a comprehensive high quality Patient Flow service in cooperation with the Home First team. They will work independently and as part of a team and be accountable for organising/prioritising own workload to ensure deadlines are achieved. To support the facilitation of the patients along a defined pathway. To ensure accurate information gathering and data input.

Be responsible for sharing relevant information within the team, making self and others aware of data related to tasks and activity where appropriate.

Collect and collate information as directed by the administration lead and Home First Bureau team ensuring accuracy and timely updates

The post holder will have excellent communication skills and will act with an efficient, professional and friendly manner to ensure that the patient, relative, colleague receives an effective, supportive and efficient service, advising on access to and use of services. Will answer telephone calls into the department in a professional and courteous manner and will use knowledge and skills to answer and deal with them.
