Job summary

An exciting opportunity has arisen for a nurse to join our Thrombosis Service Team at Wrexham Maelor Hospital. The post is advertised as a Band 6 Development post which on completion of both prescribing course and clinic competences will progress into a Band 7 Specialist Nurse role or as a Band 7 for someone who has both Thrombosis service experience with Independent Prescribing qualification.

You will take an innovative role in the management and development of nurse-led outpatient thrombosis service with a Consultant Haematologist as the clinical lead. You will join a forward thinking innovative team working across a variety of specialities across both secondary and primary care.

The DVT service runs Monday - Friday 08:30am - 17:00pm (excluding Bank Holidays)

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

As the successful post holder you will:

Coordinate the diagnosis, assessment and management of a clinical caseload of patients with suspected and actual deep vein thrombosis in an acute outpatient setting. Liaise with the multidisciplinary team to ensure the safe continuity of care throughout all health settings

Act as a resource by providing highly developed and specialist information, advice and guidance on anti-coagulation for patients, their relatives and other health professionals

Coordinate and contribute to the provision of specialist education and training for the junior doctors, nursing staff and students within the trust and for GP surgery staff

As a Band 7 you will need to have the required independent prescribing qualification to be able to prescribe medicinal products or as a Band 6 you will be supported to complete an approved independent prescribing course to achieve the required level of competence to develop to a band 7.

Work as a specialist practitioner within the site-specific nurse-led outpatient thrombosis service of BCUHB. Offer specialist care and management of Deep Vein Thrombosis (DVT) patients using an integrated care pathway; to include assessment, planning, implementation and evaluation of care in secondary as well as primary care setting, as appropriate. Demonstrate highly developed and specialised skills using own judgement regarding appropriate treatment through an evidence based process.

Act as an expert specialist in thrombosis providing clinical expertise and working collaboratively with MDT colleagues to positively influence the rapid diagnosis and quality of care for patients with suspected Deep Vein Thrombosis

Provide highly developed and specialist advice in anticoagulation therapy and provide an educational program in anticoagulation and thrombosis for all members of the MDT

Use highly specialised communication skills to provide advice, support and management of patients with thrombosis and become a point of contact and resource for MDT, patient and carers

Take a role in continuously developing the care pathway as well as initiating and undertaking research and clinical audit to inform and innovate best practice within DVT care

Identify patients that are unable to follow pathway and use own highly specialised judgement in these situations to refer to other specialities as necessary, therefore ensuring prompt referral for further investigation
